Assessment of District Student Health Services Report

Release:

Version 05.12.10.00.22, February 20, 2013 - addendum release

Change:

  1. Updated the Health Condition categories on the Range screen and on the report.

  2. On the Range screen for students, changed the default for Student Status from Active and Inactive to Active.

Reason:

  1. To meet OSPI requirements for the 2012-2013 report.

  2. To make running the report more efficient.  The option for Active and Inactive is still available.

Implementation Notes:

Because of the changes to Health Condition categories, and their alignment with Health Condition codes, previously saved selections were blanked out on saved reports and new selections for each category will have to be entered.

Version 04.12.02.00.04, March 3, 2012

Change:

Updated the logic for the Total Number of Buildings count to match CEDARS logic for the School Code data element.

Reason:

OSPI assigns a unique building number which is used to report School Code in CEDARS. The unique building code would also match the buildings reported in the Assessment of District Student Health Services report.

Implementation Notes:

In order for a building to be included in the Building Count, the option "Include this entity in the CEDARS extract" must be turned on. This option is located at Product Setup\Student Management\Entity\Current School Year\Entity Year Options.

Version 04.10.12.01.05, April 9, 2011

Change:

  1. Added a new state report to track the number of known chronic health conditions, life threatening health conditions and severity levels for Section 3 of the Assessment of District Student Health Services Report for OSPI.  Also included are a building count and a count of the total number of students in the district. The data included in this report is a snapshot as of the system run date.

  2. A check box has been added to the Health Condition screen called Life Threatening. Use this checkbox to track individual health conditions that should be reported as Life Threatening.

Implementation Notes:

A separate "Life Threatening" Health Condition code is no longer necessary. Rather, select the Life Threatening checkbox on any student health condition that should be reported in the Life Threatening column of the report.

 

When creating a template to run the report, click on the Ranges button and change the Student Status to "Active."
