1 Overview

The Workers’ Compensation Report provides information used by an employer to complete the Employer's Quarterly Report for Industrial Insurance, required by the Washington Department of Labor and Industries. This report details the hours and wages for each employee, sorted and summarized by risk classification. A recap of the rates in use for the Workers' Compensation deductions and benefits is also included.

 

The Employer's Quarterly Report for Industrial Insurance must be submitted to the Washington Department of Labor and Industries each calendar quarter. It is due by the last day of the month following the quarter ending date. This form is used to report the total payroll, as well as the number of hours employees worked during the preceding quarter, summarized by risk classification. Quarterly reports must be signed by the employer and returned with premium payment to the department.
