Administrative Information
The Student Information section contains
Legal Name: Student’s last name, first name, middle name (name.last-name, name.first-name, name.middle-name). Maximum length = 36 characters.
Other/Former Names Used: Last, first, middle name, and suffix of other/former name(s) used by the student that have been identified as names to be printed on the standardized transcript (WA001STUDENTOTHERNAMES.OTHERLASTNAME, WA001STUDENTOTHERNAMES.OTHERFIRSTNAME, WA001STUDENTOTHERNAMES.OTHERMIDDLENAME, WA001STUDENTOTHERNAMES.OTHERSUFFIXID and WA001STUDENTOTHERNAMES.PRINTONTRANSCRIPT = Y). A maximum of 4 other/former names can be printed. Maximum length = 36 characters.
District Identification Number: Student’s district code (student.student-ID). Note: This is not the student’s SSID.
Birth Date: Student’s birth date (name.birthdate).
Parent/Guardian Name(s): For Family 1/Name 1 and Family 1/Name 2 (name.last-name and name.first-name). Maximum length = 37 characters.
Graduation Date: Student’s graduation date (student.graduation-date).
State Student Identifier (SSID): Student’s state identifier (STUDENT.MN-EDE-NBR). The format is NNNN-NNN-NNN; that is, an SSID of 1234567890 is printed as 1234-567-890.
The School of Record Information section contains
School Name: School name associated with the student’s default entity (student-entity.x-default-entity = "Y"; school.school-name).
School Street Address: School street address associated with the student’s default entity (address.street-number, address.street-dir, address.street-name, and address.apt-lot if address.street-apt is not blank).
School City, State, Zip: School city, state and Zip code associated with the student’s default entity (zip.zip-city, zip.zip-state, and zip.zip-code).
School Telephone Number: School telephone number associated with the student’s default entity (school.school-phone).
School District Name: District name associated with the student’s default entity (district.district-name).
The Administrative Information section contains
Report Date: the current system date.
